State OSHA office fines hospital $75,000 for worker fatality

Minnesota authorities fined a hospital $75,000 for violations stemming from the death of an employee who was injured by mechanical file storage equipment.
Rice Memorial Hospital in Willmar, MN, where the incident occurred in November 2007, plans to appeal the fines, according to the West Central Tribune in Willmar. The victim suffered injuries after getting caught in the automated equipment, which was in the medical records department, the newspaper reported.
Minnesota’s state Occupational Safety and Health Administration issued the penalties for violations under the following standards, according to OSHA inspection information:
- General requirements for machinery and machine guarding (federal 1910.212)
- Alteration of tools and equipment (Minnesota state standard)
- Motor start button (Minnesota state standard)
